What is required to obtain an ID card?
ORIGINAL birth certificates, passports of parents or one of them are submitted for the first time when the child reaches the age of 14.
* Certificate of registration of the place of residence of the established model (Appendix-13) or an extract from the register of the territorial community, issued through the «ACTION» application — if available;
* Identification number — if available.
To exchange a 1994 passport of a citizen of Ukraine for an ID card:
- Passport of a citizen of Ukraine
- * identification number — if available
- * on marriage and divorce — a marriage certificate, a divorce certificate, a court decision on divorce that has entered into legal force, or a document issued by the competent authorities of a foreign state. Such documents are submitted with a notarized translation into Ukrainian.
